I am working with someone who has these and describes them as radiating outward from his testicles. I have very little understanding of them so would love to hear anyone's experience.

Has he seen a medical doctor for this yet?  I would definitely get that out of the way first.  It could be a number of things. I have known two people who have had those sharp radiating pains from that region later to find they were testicular growths.  One of them was benign and the other turned out to be testicular cancer.  Both of them had it in their early 30's.  I also have another good friend who reported the same thing and it ended up being a kidney stone. Kidney stones are normally very painful.  The other possibility is that it can be a hernia which is common as well.  But like I said, I am just guessing here from the experiences of people I know. I would tell this person to go see a medical doctor to make sure it isn't something serious.
